- People living with psychosocial disability
- People leaving hospital or custody
- Anyone ready for a fresh start with a new coordinator

Experts in hospital discharge and transitions from custody
Leaving hospital or custody is one of the riskiest moments in a person's life. We plan early with discharge planners, mental health teams and corrections staff, so housing, providers and clinical follow-up are ready from day one, and we stay closely involved through the first 90 days.
Starting with us is straightforward.
Four simple steps from first phone call to fully supported.
Say hello
Call, email or refer online. We'll arrange a no-pressure first conversation at a time that suits you.
Share your plan
If you're comfortable, share your NDIS plan so we understand your goals, funding and preferences.
Agree the supports
Together we put a simple service agreement in place: what we'll do, when, and how we'll stay in touch.
Move forward
We connect you with the right supports, check in regularly and adjust as life changes.
Where we work
Our head office is in Melbourne, and we support participants across two states.
Good to know before you start.
What happens in the first chat?
We'll ask about you, your goals and what's happening now. If you're comfortable, we'll look at your NDIS plan together and explain what each part means. There's no obligation to go ahead.
What is a support coordinator?
A support coordinator helps you put your NDIS plan into action: explaining your funding, finding providers, setting up agreements and sorting out problems. Compare the levels.
Can you help someone leaving hospital or custody?
Yes, it's one of our specialties. We plan early so housing and supports are ready on day one. Read about transitions.
Which areas do you cover?

Referring a participant? Ready when you are.
