How we work across both states
You choose how we keep in touch, and you can mix and match.
Face-to-face
At home, at a local café, on a hospital ward or wherever you feel comfortable.
By phone
Quick check-ins, updates and problem-solving, without needing to travel.
By video
Great for care team meetings, regional areas and anyone who prefers not to travel.
In both states we work closely with hospital discharge teams, mental health services, corrections staff and community corrections, so people leaving hospital or custody have supports ready from day one. Read about our hospital and justice transitions work.

Do you cover my suburb?
We focus on the regions listed on this page and can often support people further afield by phone and video. Call 1300 941 734 and we'll confirm.
Do I have to meet in person?
No. You can choose face-to-face, phone or video, or a mix. Many people meet in person at the start and then keep in touch by phone.
Can you support people in regional areas?
Often, yes. We combine video and phone contact with planned visits, and we work with local providers and services near you.
