- The service you're referring for, or 'not sure'
- Any discharge or release date, and other key timeframes

What happens after a referral
We confirm it's received
A member of our team confirms we've received the referral and asks any follow-up questions.
We contact the participant
We arrange an introductory conversation with the participant and, if they like, their family or nominee.
We agree the supports
Once the participant is happy to go ahead, we put a clear service agreement in place.
We keep you in the loop
With the participant's consent, we update you on how the referral is progressing.
Send us the details
Use the form, or email the referral to admin@splendidcoordination.com.au. For urgent referrals, please also call 1300 941 734.
Before you refer
Please make sure the participant (or their nominee) knows about the referral and has agreed to their details being shared with us.

Hospital discharge teams
Early, joint planning for long-stay patients, with supports and follow-up confirmed before the discharge date.
Mental health services
We work alongside case managers and clinicians, sharing information with consent so care stays joined up.
Justice and corrections
Pre-release planning that works around parole and community corrections requirements.
Plan managers
Clear budgets, timely service agreements and early warning if funding is at risk of running out.
Allied health and providers
A coordinator who answers, follows up and keeps everyone working from the same plan.
Questions from referrers
Is there a cost to refer?
No. There's no cost to refer and no obligation for the participant to go ahead. If they do, our services are paid from their NDIS plan.
Does the participant need to agree to the referral?
Yes. Please make sure the participant, or their nominee, knows about the referral and has agreed to their details being shared with us.
What if the participant doesn't have support coordination funding yet?
Refer anyway, and tell us. We can talk with you about the evidence needed and how to ask for it at a plan reassessment.
Can you take urgent referrals?
Yes. Mark the referral as urgent and call us on 1300 941 734 as well. We prioritise imminent discharges, releases and situations where safety is at risk.
Will you keep me updated?
Yes, with the participant's consent. We'll confirm when we've made contact, when supports are agreed, and if anything significant changes.
Prefer to talk it through?
Call us and we'll take the details over the phone. We reply to every referral within one business day.
